1.  Please arrive 15 minutes prior to your scheduled appointment time so that we can register your medical, dental and personal information prior commencement of your exam.

2.  Bring the names and doses of any medication you are taking.

3.  Bring the names and addresses of your medical physicians.

4.  Bring all pertinent dental insurance information.

5.  If you have had dental x-rays in the last 2 years please obtain copies from your previous dentist or let us know in advance so that we can request copies and have them in the office prior to your appointment. 

6.  Your first appointment will last approximately 1.5 hours.  During this visit we will obtain a full set f x-rays, a tooth by tooth charting of existing restorations, a periodontal (gum) exam, intraoral photographs, a TMJ (temporomandibular joint) screening, an oral cancer screening, a health history and dental history screening and a present dental needs assessment.

7.  Your teeth will not be cleaned on this first visit, rather at completion of the exam the doctor will give you a general verbal assessment of your dental needs.  A written treatment plan that includes specific needs, fees, number of appointments and insurance estimates will be prepared for presentation at your second visit.  At the second visit your teeth will be cleaned and reassessed to determine appropriate recare interval (3 - 6 months).
